Nitram in critical care
Just got off the phone with Tev and she asked me to pass the latest up to the board. Nitram's just been moved up to critical care, between the possible heart murmur and the fact that he hasn't urinated in something like twenty hours, the doctors are apparently, in Tev's words, panicking. I'll keep tossing up updates as I hear more.

It doesn't really need to be said, but I'm sure we're all thinking about them. These kinds of situations are frustrating, if only because there's little that can be done from our end. If anyone's due a break, though, it's Tev & Nitram.

As WhiteHaven has said, Nitram's been moved to the Critical Care Unit (CCU). The reason? His blood pressure is still too low. He's also not produced the urine that should show up after 5-6 units of Saline through the IV. If he's not pissed by 1am EDT, he gets a catheter.
They kicked me out of CCU, it closed at 11p. He does get to keep his LapTop, so he might post, but he's got an IV in each arm, so don't expect much. When I left, his blood pressure was 101/39. (FYI: the 101 is 'normal', but the lower number should be doubled, at least). When I took him in at 3:30am Friday morning, it was 66/23, and he had a 100+ fever. His fever had dropped when they gave him Motrin, but it had started to creep up slowly again by the time I left.
The doctors are treating him as if he's Septic, because the low blood pressure/high heart rate/fever/etc all tend towards that diagnosis. Even if he's not, they're going to hit him with Antibiotics and have him in an area where he can get one-on-one care.
So far, he's in no pain (other than the IV stabs and other indecencies). There's no vomiting, no diarrhea, no symptoms of flu or other virus. They are going to give him Albumen next (after 2 units of blood) to try to thicken his blood and get the blood pressure raising to a more normal level. They are still giving him Saline, as he was extremely dehydrated, to the point they blew out 3 veins getting the second IV in his arm.
OH, and he's got a heart murmur that they're investigating, possibly from the low blood pressure, possibly from complications of his liver disease. The way it was explained was the blood can't flow thru the liver from the scarring, so it backs up (portal hypotension). Portal Hypotension leads (somehow) to a buildup of free-flowing fluid in the abdomen (ascites), causing bloating, dehydration, and other problems. The Ascites and the Hypotension cause more backups and pressure problems, and starts to cause damage to other organs (Enlarged Spleen, Heart Murmur, "Right Side Heart Failure", etc)
The treatment thus far:
1. 2 units of Blood, more possibly on the way if the Hemoglobin count is still low.
2. On-going regimen of IV antibiotics, to treat whatever infection might be there
3. UltraSound examination of the Heart and Abdomen, as well as X-rays.
We have already been told a sample of Ascites Fluid will be drawn from his abdomen for Culturing (seeing if anything is growing in it. Last time they found Anorexic Bacteria there). The date for this was not set, but Culture Growth takes 24-48hrs. Therefore he will be in the hospital a minimum of 4 days.
And that is all I have for you.

Morning Update:
They will be doing Paracentesis (drainage of ascites) sometime today, to relieve some of the pressure in his abdomen. There is a chance the IV fluid is seeping out of his veins and gathering there, instead of going thru his kidneys. While they are draining him, samples will be collected for Culture tests (seeing if anything infectuous is growing there).
Now i'm off to fetch his mum from the airport. I'll update later today.
