Transporter tech is the only thing the Feds have going for them. Replicator tech is way too small-scale, compared with titanic Imperial industrial capability; for fuck's sake, they can build small planets, in six months, in secret, and with no discernable strain on the galactic economy.

Regarding medical tech, the mere fact that vader has a metal spine shows that Imperial med-tech outstrips Federation technology.
