- No crosshairs. You either fire from the hip or use the iron sights. Very cool.
- No money. Instead of choosing your armament based on the money you get from fragging and your team winning, you have a set configuration based on a balance of weight and speed. The more stuff you carry around, the slower you are. You can change this configuration any time, and it takes effect at respawn. This eliminates the ever-recurring CounterStrike scenario of one team winning 3 or 4 times in a row and then continuing to dominate because the other team can't afford decent weapons.
- Bandaging. If you're hit badly enough, you have to sit down and bandage yourself, or you'll bleed to death.
- Heartbeats after action. After an exchange of fire, you'll hear your heart beating while the adrenaline rush cools down.
- Duck to pick up items. Just running stuff over doesn't cut it anymore.
- Server-optional player identification. Some servers have the name identification on HUD's disabled, so you have to rely on your perception to tell who's on your team. This would be cooler if they distinguished the team skins more. They look way too similar at the moment for this to add a positive part to the experience. If the team skins were more easily differentiable, it'd be more fun and exhilarating.
- Flash-bang effects. The flash-bang doesn't just blind you; it deafens you as well. You hear a ringing noise when one goes off near you that stays in your ears for 10 seconds or so.
- When you're dead, you can't move your camera around to track the guy who killed you. You're dead.

Recommendation: TrueCombat (Quake 3 mod)
I highly recommend this mod for Quake 3. It's called TrueCombat. I know lots of people are thinking, "Oh god, not another CounterStrike clone," but I like this much better than CounterStrike. It's brutally realistic, and here's what that entails:

That's all been done before, but I've never seen it on Quake3, so it might be okay
EDIT: There was another game, kinda cool, but bombed because everyone was too busy playing CS. I can't remember name, but it was about the best game ever if you are looking for "realism". I think it had something to do with the word "ops", but it wasn't Covert Ops... i think... damnit what was the game...
