yeah like 1920's mobsters even with a communicator are just too far behind in tech to be a concievable theat to an advaced space faring civilization....
They were a threat to Kirk and Co. when they were on the ground--not surprising, since they had submachineguns and numbers. And I don't think Kirk was being really serious when he said the Iotians would want "a piece of our action". To me, that sounded like, "Well, we just fucked with their culture again, and before too long we're going to have to deal with them pushing out into space." No threat to the Federation, but they could be bothersome to their neighbors.
By the way, wouldn't it have been cool if some minor character from TNG, DS9, or VOY, maybe someone aggressive but not too bright like a security redshirt, turned out to be an Iotian who joined Starfleet after his home planet developed warp drive?
The Star Trek 25th Anniversary NES game follows up on A Piece of the Action a bit at the end. Apparently, the Iotians used the technology in the communicator to wipe themselves out (not intentionally, of course) and Kirk and co had to go back in time a year or two to recover it.
Of course, I seriously doubt that the NES game could be considered canon.
It was a pretty good game, though. Not as good as the 25th Anniversary PC game by any stretch, but still not half bad.

Originally, the DS9 30th anniversary show, Trials and Tribble-ations was going to feature the DS9 crew, on a relief mission come across the Iotians. The good news is, they're no longer gangsters. The bad news its that they're now paying tribute to Kirk, Spock, and the ENT, going around in 23rd century drag.
In the end, they felt it was better to just do the Tribbles, though I would have liked to have seen something like this done.
