Never on the E-D. The War Galaxies had the upgrades, but the E-D never did (it might have been the stupid, senseless loss of the E-D that prompted the redesign--though that didn't prompt a court-martial for Riker, so maybe not).

It is possible the E-D had a power upgrade but not a saftey one. The incident in question for the power comparisons is about how long the warp core can last before its been battered down. It seems to suggest raw power more then anything else. The E-D had a power upgrade, but saftey still wasn't the best. The saftey problem was fixed with the War Galaxy upgrades.

Considering that the E-D was hit several times during that time loop and was destroyed each time, I can't see how it would be a "one-in-a-million shot." Several minor things had changed each iteration through the loop, so if there was a very small chance that hitting the E-D's nacelle would cause an explosion, then there should've been iterations where the E-D was hit but circumstances were just different enough for it to survive.

However, since the E-D died each time, it appears the nacelles were quite vulnerable on the E-D at that time.

Also, remember TNG Children of Darmok. The cruiser specifically targetted the nacells of the E-D and nothing else. It was pretty clear who was going to win that battle if it would have played out.
