So each program has to state what is replicated and what is not?

I imagine there is a default rule set which can be altered within reason.

Maybe when the safeties are off (which I believe they were with the paper example) as many items as possible are replicated (to give it that extra realism).

Well, that makes sense then... I guess the holodeck also serves as a giant replicator.

Also, Picard may be fully aware that the computer will not replicate certain objects unless a real individual attempts to carry them out. He threw the book outside the holodeck construct. There was no real person taking it off the holodeck and the computer would interpret that as a object that does not need to be replicated. After all, a holographic person could've also thrown that book towards the arch exit.
