The same plot could have been used for a comedy.

I'd say it's just another movie making use of the "creepy child" + "creepy doll" tropes. In Child's Play, it was an adult that transferred his consciousness into the doll to escape his death/evade capture. Here, it seems like either something the child didn't want to have happen to them, something done because the child met an untimely death, or perhaps some sort of punishment doled out due to the child being "rotten" or bad in some way, (kind of like a reverse Pinocchio).
Of course, the protagonist(s) will deny that anything supernatural is going on until it's too late, and the doll/boy will exhibit stealth, strength, or other characteristics to make even professional spys and assassins jealous.

I am guessing that the potential nannies did just this when they noticed something odd about the doll.
